Facial Rejuvenation

As we age, the face changes on several levels – the quality of the skin, the amount and distribution of subcutaneous fat, facial muscle activity, supporting ligaments, and the underlying bony structure all gradually change. Facial rejuvenation is therefore not a single treatment, but an individualised approach in which we select one or more methods according to facial anatomy, skin quality, the degree of age-related changes, and your individual goals. Our aim is not to change your facial features, but to maintain or restore a natural, fresh and well-rested appearance.

About facial rejuvenation

We cannot stop the ageing process, but with appropriate treatment we can address many of its visible effects.

Facial rejuvenation is not simply about treating an individual wrinkle. We assess the face as a whole and determine which changes contribute most to an aged or tired appearance.

We most commonly address:

  • expression lines and wrinkles,
  • volume loss,
  • changes in facial proportions,
  • reduced skin firmness and elasticity,
  • changes in skin quality and texture,
  • pigmentation irregularities and photodamage,
  • scars and enlarged pores,
  • descent of soft tissues,
  • excess skin of the face, eyelids or neck.

Different methods address different components of ageing. For this reason, the most natural results are often achieved through a carefully planned combination of treatments.

Botulinum toxin – reducing expression lines

Botulinum toxin is primarily used to soften wrinkles caused by repeated contraction of facial muscles.

The most commonly treated areas include:

  • forehead lines,
  • vertical lines between the eyebrows,
  • wrinkles around the eyes,
  • selected expression lines in the lower face,
  • prominent platysmal bands in the neck.

The aim is not to completely eliminate facial expression, but to selectively reduce excessive muscle activity while preserving a natural appearance.

Dermal fillers – restoring volume and facial contours

As we age, the amount and distribution of facial fat gradually change, and so does the underlying bony support of the face.

Hyaluronic acid dermal fillers can be used, when appropriate, to:

  • restore lost volume,
  • provide additional support to soft tissues,
  • enhance the cheeks,
  • shape or correct the chin,
  • improve jawline definition,
  • shape the lips,
  • soften selected hollows and facial transitions.

In younger patients, fillers are often used primarily for facial harmonisation and proportion, while in older patients they may also be used to compensate for age-related volume loss.

It is important not to overcorrect the face with excessive volume. The aim of rejuvenation is not to make the face larger or overfilled, but to preserve its natural proportions.

Radiesse® – tissue support and biostimulation

Radiesse® contains calcium hydroxylapatite and differs from conventional hyaluronic acid fillers in part because of its biostimulatory properties.

Depending on the way it is used, Radiesse® may be incorporated into treatment when we wish to improve tissue support, enhance facial contours, and stimulate a gradual improvement in skin quality.

The choice between a hyaluronic acid filler, Radiesse®, or another treatment depends on facial anatomy and the individual treatment goal.

Skin biostimulation

A fresh and rested appearance does not depend only on facial volume and contours. Skin quality is equally important.

To improve hydration, structure, elasticity and firmness, we use different biostimulatory and regenerative treatments.

At Klinika Višnjar, these include:

  • Sculptra® – to stimulate the production of the patient’s own collagen,
  • Profhilo® – for bioremodelling, hydration and improved elasticity,
  • Sunekos® – a combination of hyaluronic acid and amino acids designed to support the extracellular matrix,
  • Rejuran® – polynucleotides used to support skin regeneration and improve skin quality.

Each product has different characteristics and mechanisms of action. There is therefore no single biostimulatory treatment that is best for every patient.

PRP – regeneration with your own platelet-rich plasma

PRP (Platelet-Rich Plasma) offers a different approach to skin regeneration.

It is an autologous biological preparation obtained from the patient’s own blood. Centrifugation is used to prepare plasma with a higher concentration of platelets, which contain numerous growth factors and signalling molecules.

These molecules are involved in fibroblast activity, collagen production, angiogenesis and other processes of tissue repair.

PRP does not add volume and does not alter facial features. The aim of treatment is to support the skin’s natural regenerative processes and overall quality.

PRP may be used on:

  • the face,
  • the eye area,
  • the neck,
  • the décolletage,
  • the backs of the hands.

It may be performed as a standalone treatment or incorporated into combined protocols with microneedling or laser procedures.

Medical microneedling

During medical microneedling with Dermapen 4, numerous controlled microchannels are created in the skin, stimulating natural healing and remodelling processes.

Microneedling is commonly used to improve:

  • skin texture,
  • fine lines,
  • enlarged pores,
  • uneven skin appearance,
  • selected acne scars,
  • overall skin regeneration.

Depending on the indication, microneedling may also be combined with other regenerative treatments, including PRP.

CO₂ laser skin rejuvenation

For more pronounced surface changes, fractional ablative CO₂ laser treatment may be appropriate.

The laser creates microscopic areas of controlled injury within the skin, triggering a more intensive healing response and collagen remodelling.

It is commonly used for:

  • fine and moderately pronounced wrinkles,
  • photodamaged skin,
  • uneven skin texture,
  • enlarged pores,
  • selected pigmentation irregularities,
  • acne scars.

CO₂ laser treatment is more intensive and requires more downtime than most injectable or less invasive regenerative procedures. We therefore recommend it when the expected benefit justifies a more intensive treatment and recovery period.

Chemical peels

Chemical peels primarily act on the superficial layers of the skin and, depending on the type and strength of peel, may also affect somewhat deeper layers.

They can be used to improve:

  • dull and tired-looking skin,
  • uneven skin tone,
  • superficial pigmentation,
  • fine lines,
  • skin texture.

The type and depth of peel are selected according to skin type, indication and desired treatment intensity.

Non-surgical thread lift

When mild to moderate descent of the soft tissues is present, a thread lift with absorbable threads may be suitable for selected patients.

The threads provide mechanical support to the tissues and also trigger a biological tissue response over the following months.

The method is primarily suitable when a moderate lifting effect is desired without surgery.

In cases of significant tissue descent or substantial excess skin, threads cannot replace a surgical facelift.

Surgical facial rejuvenation

Non-surgical treatments have their limitations.

When ageing results in more pronounced soft-tissue descent, excess facial or neck skin, or significant changes around the eyelids, surgery may provide a more durable and anatomically appropriate result.

At Klinika Višnjar, we perform several surgical facial rejuvenation procedures.

Facelift and neck lift

A facelift is designed to reposition descended facial soft tissues.

Modern facelift surgery is not simply about tightening the skin. The deeper facial structures that play an important role in ageing are also addressed.

The procedure may be combined with a neck lift.

In appropriately selected patients, we also perform a deep-plane facelift, in which the deeper layers of facial tissue are repositioned.

Blepharoplasty – eyelid surgery

The area around the eyes is often one of the first parts of the face to show signs of ageing.

Upper or lower blepharoplasty can be used, depending on the anatomy, to remove or redistribute excess skin and fatty tissue and improve a tired or heavy appearance around the eyes.

Blepharoplasty may be performed as a standalone procedure or combined with other facial rejuvenation treatments.

Brow lift

As the eyebrows descend, the appearance of the upper third of the face may change and apparent excess skin above the eyes may become more pronounced.

When appropriate, a surgical brow lift can improve eyebrow position and create a more open appearance around the eyes.

Facial rejuvenation with fat transfer

A patient’s own fat may also be used in selected cases to restore facial volume.

Fat is harvested from another area of the body, appropriately processed, and transferred to selected areas of the face.

Fat transfer may be performed as a standalone procedure or as part of combined surgical facial rejuvenation, for example alongside a facelift.

Why do we combine different treatments?

Different procedures address different components of facial ageing.

Botulinum toxin primarily affects facial muscle activity.

Dermal fillers and fat transfer restore or reshape facial volume.

Biostimulatory treatments and PRP primarily target skin quality and regenerative processes.

Microneedling, chemical peels and CO₂ laser treatments address skin texture and surface changes.

Threads can provide a moderate mechanical lift of the soft tissues.

A surgical facelift allows repositioning of descended deeper structures and removal of excess skin – something injectable treatments cannot achieve.

For this reason, comprehensive facial rejuvenation is not based on choosing the most popular treatment, but on identifying the actual cause of an aged or tired appearance in each individual face.

Facial rejuvenation at different stages of life

Age alone does not determine which treatment is appropriate.

Facial ageing is strongly influenced by genetics, anatomy, sun exposure, smoking, changes in body weight, lifestyle and many other factors.

The following age groups should therefore be regarded only as a general guide.

Facial rejuvenation in the 20s and early 30s

At this stage, treatment is usually not focused on correcting pronounced signs of ageing.

The main considerations may include:

  • skin quality and skincare,
  • prevention of photodamage,
  • treatment of acne scars,
  • improvement of skin texture,
  • correction of congenital or anatomical facial disproportions.

When clearly indicated, botulinum toxin may be used for pronounced expression lines.

In younger adults, dermal fillers are more commonly used to harmonise facial proportions – for example the lips, chin or cheeks – rather than to replace age-related volume loss.

Microneedling, chemical peels, PRP or selected biostimulatory treatments may be considered when the aim is to improve skin quality or address a specific indication.

Facial rejuvenation between approximately 30 and 45

During this period, several components of facial ageing often become more apparent at the same time.

These may include:

  • more pronounced expression lines,
  • early volume loss,
  • changes in the under-eye and mid-face area,
  • reduced jawline definition,
  • declining skin quality and elasticity,
  • photodamage and pigmentation.

A combination of approaches is therefore often appropriate.

Botulinum toxin may be used for expression lines, fillers or biostimulators for volume and tissue support, PRP, Profhilo®, Sunekos®, Rejuran® or Sculptra® for different aspects of skin quality, and microneedling or laser treatment for texture and surface changes.

In selected patients with early soft-tissue descent, thread lifting may also be considered.

Facial rejuvenation after 45

With age, changes in the deeper facial structures often become increasingly important in addition to skin quality.

We more commonly see:

  • loss or redistribution of facial volume,
  • descent of the cheeks,
  • reduced jawline definition,
  • laxity of the face and neck,
  • changes of the upper and lower eyelids,
  • eyebrow descent.

Non-surgical procedures continue to play an important role, particularly for expression lines, skin quality and maintenance of results.

However, when tissue descent and excess skin become more pronounced, repeatedly adding larger amounts of filler is not necessarily the appropriate solution.

At this stage, surgical options such as facelift and neck lift, deep-plane facelift, blepharoplasty or brow lift may be considered.

Surgical and non-surgical treatments are complementary rather than mutually exclusive. Surgery can reposition tissues and remove excess skin, while botulinum toxin, biostimulation, PRP, laser treatments and other procedures can continue to address facial expression and skin quality.

Natural results are more important than any single treatment

The aim of aesthetic rejuvenation is not to erase every sign of ageing.

Facial lines, expressions and individual features are part of a person’s identity.

Our goal is for the face to appear fresh, rested and harmonious, rather than changed or unnatural.

Treatment planning therefore does not begin with the question “Which product do I want?”, but with which facial changes should be improved and which method can address them most appropriately.

Which facial rejuvenation treatment is right for you?

There is no universal facial rejuvenation plan.

During your consultation, we assess facial anatomy, skin quality, facial proportions, muscle activity and any soft-tissue descent, while also discussing your wishes and expectations.

Based on this assessment, we prepare an individual treatment plan that may include a single procedure or a combination of surgical and non-surgical methods.

The goal is always the same: a natural result that preserves your individual facial features while improving freshness and facial harmony.
